This is the blank addendum for TLI service signup. It contains no customer data and is not a record of anyone’s consent. Print it, complete it if you choose to enroll, and return it with your service agreement to your TLI representative. This page does not submit or enroll you online.

Confirmation before alerts

After TLI receives your completed consent addendum, the confirmation message is:

Third Level IT LLC: Reply YES to confirm service and website lead alerts at this number. Frequency varies; msg & data rates may apply. Reply STOP to cancel or HELP for help. Alerts begin only after YES.

Alerts remain disabled until a YES reply is received and recorded. TLI retains the agreement date, mobile number and YES reply timestamp with the owner’s CRM record. If you previously stopped messages, follow the START instructions in the opt-out confirmation and contact TLI to verify re-enrollment.
